Description:

We are a growing private practice needing to partner with a music therapist to serve our growing wait list! This work includes providing in-client home or facility-based therapy in the areas requested by the contractor (MMT has substantial client wait lists in South Bend Indiana or Indianapolis Indiana), as well maintaining contract-appropriate client documentation.

Our agency provides music therapy services under the Medicaid Waiver program in Indiana and also State-wide Hospice contracts. Therapists may specify in their contract which type (or both) of work they prefer, control their own schedule, referrals and caseload, set their own rate of pay, and work with Mainstay to create the terms of the contractual agreement.

Interested therapists must be knowledgeable in the integration of music therapy intervention and techniques specific for persons with developmental disabilities and neurological and other impairments inclusive of: cerebral palsy, vision impairments, Autism, Down Syndrome, dual diagnosis, seizure disorders and/or hospice populations and be able to comply with contract specific documentation and personnel standards.

Mainstay fully understands the differences between being a hired employee (W2) and an independent contractor (1099). With this understanding, we work with contractors who are completely autonomous from the central functioning of our agency.

Job Duties:

Provide quality in-home or clinic-based services to the client weekly; conduct assessments; develop treatment plans; maintain weekly progress notes; submit quarterly progress reports; maintain communication with family and team members; create and maintain a reliable schedule with clients; maintain personnel records as mandated by MMT contracts; retain client files in concordance with contract mandates.
