Description

This was created to help people who have communication differences let providers, teachers, and other caregivers know how they communicate best. Often when we start working with someone new it takes time for them to learn how the individual communicates. This can be a frustrating time for the individual as they are not always understood. The hope is that this bridges the gap and gives providers a step in the right direction to understanding the individual faster.

There is a section for people who primarily use their voice to communicate as well as sections for those who use Augmentative Alternative Communication. The last pages hold permissions and requests for families as well. Use this as paperwork for the family to fill out for you to get to know the student/ client better as well as help inform you of things needed for their device.
